The nasal base is the foundation of the nose, affecting not only nasal breathing but also appearance. A stable nasal base can ensure a solid foundation for the lower third of the nose. Unstable nasal base may result in ptotic or underprojected nasal tip, columellar retraction, overly acute nasolabial angle or short nose especially after augmentation rhinoplasty procedure. Stabilizing the nasal base is critical to providing durable support of the nasal tip against forces of scar contracture, gravity, reaction force, and facial musculature.
In Asians, the nasal bases are commonly unstable. Therefore establishment of a stable nasal base is very crucial in Asian rhinoplasty.
We prefer to use autogenous cartilage to restore the nasal base stability. The second choice is irradiated homologous costal cartilage. We avoid using porous high-density polyethylene (medpor) because of its high risk of infection.
There are many techniques for restoration of stable nasal base. The author prefer the following methods:
1. If caudal septum is stable, autogenous cartilage extension graft with or without splinting graft
2. If the caudal septum is unstable or deviated, we choose caudal septal advancement with extended spreader grafts using either autogenous cartilage grafts or homologous costal cartilage grafts
Nasal base stability is a critical key factor of successful Asian rhinoplasty. Detailed analysis and diagnosis of unstable nasal base are crucially important. We recommend that rhinoplasty surgeons must be familiar with variable techniques in restoration of stable nasal base
